  1. For PRE SCHOOL, the child must be (4) four years old by the end of October as per DepEd Order No. 47, s. 2016.


    For KINDERGARTEN, the child must be (5) years old by the end of October as per DepEd Order No. 47, s. 2016.

    Interested applicants must register and submit the following before taking the assessment:

    • Photocopy of NSO Certificate of Live Birth
    • Testing fee receipt
    • 1 (1) copy of the most recent colored ID picture (2×2) with white background
  1. Submit the following before taking the examination:

    • Certificate of Live Birth from PSA (photocopy)
    • Testing Fee of P100.00 (non-refundable)
    • Photocopy of report card with LRN number for Grade 1
    • Photocopy of report card with a general average of 85% or above to be submitted after the 4th grading period for Grade 2 – 6
    • One copy of the most recent colored ID picture (2×2) white background
  1. Submit the following documents to the Guidance Office.

BASIC REQUIREMENTS FOR INCOMING GRADE 7:

  1. Properly accomplished Admission Test Application Form from BES Guidance and Counseling Center
  2. Clear photocopy of PSA-issued Birth Certificate
  3. Clear photocopy of 1st grading Report Card (Form-138) with a general average of 80%
  4. Two (2) pcs. of latest colored I.D. picture (2”x2”) with white background and name tag

 

BASIC REQUIREMENTS FOR TRANSFEREES (GRADES 7 and 8):

  1. Original Copy and Photocopy of Report Card with at least 85% General Weighted Average
  2. Properly accomplished Admission Test Application Form from BES Guidance and Counseling Center
  3. Clear photocopy of PSA-issued Birth Certificate
  4. Two (2) pcs. of latest colored I.D. picture (2”x2”) with white background and name tag

 

  1. After submitting the needed documents, the Guidance Counselor will give the schedule of the examination of the test taker through the Test Permit.

 

  1. Once the permit is given to the test taker/guardian, the amount of P100.00 will be paid to the Accounting Office and it is non-refundable/non-transferable. 
  1. Incoming Grade 11 students who have completed the Junior High School program can apply and are eligible to take the USL – Senior High Admission Test.
  2. All Grade 10 USL students and Grade 10 students from Science High School/Science section are exempted from the test.
  3. Exempted applicants are eligible to proceed for Pre-enrolment.
  4. All applicants for the test must have a general average of at least 80% with no failing marks in any of the grading periods.
  5. Students are allowed to take USL – Senior High Admission Test once only.
